UTICA, N.Y. — Excellus BlueCross BlueShield (Excellus) says nonprofit organizations in its 14-county Utica region can apply for Fall 2016 Community Health Award grants, with the deadline a week away.

The deadline to submit an application to be considered for a Community Health Award is Oct. 18, the health insurer said in a news release.

The Excellus Utica region includes Oneida, Madison, Herkimer, Jefferson, Lewis, St. Lawrence, Clinton, Delaware, Essex, Franklin, Fulton, Hamilton, Montgomery, and Otsego counties.

Organizations can apply for an award of up to $4,000. They can use the award for programs that have “clear goals to improve the health or health care of a specific population,” Excellus, Central New York’s largest health insurer, said.

It will consider programs that improve the health status of the community, reduce the incidence of specific diseases, and promote health education and further overall wellness.

Applications and additional information are available online by clicking here.

Excellus will not consider applications received after 11:59 pm on Oct. 18. The health insurer will announce award winners in November or December.
