MVHS launches new services to aid patients with chronic conditions

UTICA, N.Y. — The Mohawk Valley Health System (MVHS) has launched new chronic-disease-management and specialty pharmacy services for patients living with chronic and complex conditions.

The services are offered in partnership with Trellis Rx, LLC, which is part of the CPS Solutions, LLC family of companies.

Trellis Rx specialty pharmacists and patient liaisons will be embedded in MVHS care teams to collaborate with providers to ensure patients receive the support needed to manage complex medication regimens.

The integrated approach improves medication access, increases adherence, and streamlines the care experience for patients with serious conditions like cancer, HIV, heart disease, and diabetes, the organizations say. It can lead to better patient outcomes and quality of life.

“For so many people in our region, living with a chronic condition means dealing with extra burdens like juggling appointments, managing expensive drugs, and working through complicated paperwork and approvals,” MVHS Director of Pharmacy Services Christopher Houle said in a press release. “Patients and their loved ones have enough to worry about. These new services and caring team members will give patients and their loved ones much-needed support so they can focus on what matters most – the healing journey.”

MVHS includes St. Elizabeth Medical Center, Faxton St. Luke’s Healthcare, MVHS Rehabilitation and Nursing Center, Visiting Nurse Associate of Utica and Oneida County, and Senior Network Health. It will open the Wynn Hospital in 2023. MVHS employs more than 4,200 full-time equivalent employees.
